Outreach services.
Support that comes to you — in your own home and your own community, for as many or as few hours as you actually need.
Support that fits around a life, not the other way round.
Not everyone needs, or wants, a full supported-living package. Our outreach service provides support in someone's own home and out in the community — from a few hours a week to daily visits.
It suits people who are living independently and want to keep it that way, people stepping down from a more intensive package, and people whose family carers need reliable, skilled support alongside them.
What the hours are used for is entirely up to the person: getting to appointments, managing a tenancy, budgeting, cooking, college, work, seeing friends, or simply having someone consistent to check in with.
Outreach, in practice.
The same standards as our supported living, delivered in someone else's home.
Consistent staff
The same faces.
We keep outreach teams small and consistent, because trust is the thing that makes short visits actually work.
Goal-led
Hours with a purpose.
Each package has goals the person set themselves, reviewed openly — so it is clear whether the support is doing what it was meant to.
Flexible
Scales up and down.
Hours can increase through a difficult period and reduce again as someone gets back on their feet. The package follows the person.
Would a few hours a week make the difference?
